5. Patient Responsibilities
As a patient, you agree to:

- Follow Recommendations: Follow the treatment plan and recommendations provided by your chiropractor to the best of your ability

6. Fees and Payment

If you are bringing a minor (under 18 years old) for treatment, you represent that you are the parent or legal guardian with authority to consent to treatment on behalf of the minor. By providing consent, you agree to be financially responsible for all services provided to the minor.
In cases of separated or divorced parents, we require written authorization from both parents or a court order specifying custody and medical decision-making authority.
