5. Patient Responsibilities

As a patient, you agree to:

• Follow Recommendations: Follow the treatment plan and recommendations provided by your chiropractor to the best of your ability

6. Fees and Payment

If you have insurance coverage, we will make reasonable efforts to verify your benefits and file claims on your behalf as a courtesy. However, you are ultimately responsible for payment of all services received, regardless of insurance coverage or reimbursement.

In cases of separated or divorced parents, we require written authorization from both parents or a court order specifying custody and medical decision-making authority.
